That code looks fine to me. Does the mysql user rsmartap_php have permissions on rsmartap_encuesta? Are you using the correct password for rsmartap_php?

Telnet can actually be used on any port. You can troubleshoot all types of servers, such as http by using telnet on port 80. Telnet should never be used to transmit any kind of password or sensitive information though as it's ridiculously insecure.

Each time you need a .war deployed place the .war file in /home/sms/filename.war, and make a new post in customer stating your username, and the filename.

You know you can do this yourself, right? Delete everything above DROP TABLE IF EXISTS `mybo_strikes`; and everything below the last INSERT INTO `mybo_strikes`... and then import that .sql file. Anyways, I did it for you. How does your database look now? -
It doesn't look like you completed the three simple steps of the dedicated IP purchase process. The instructions on how to get a dedicated IP are posted here http://www.heliohost.org/home/signup/217 and http://www.heliohost.org/home/signup/218 and http://wiki.helionet.org/Using_SSL and dozens (if not hundreds) of places on these support forums if you had searched. Our dedicated IP purchase process currently does not differentiate between donations and purchases. The only way we know what payment belongs to what account is by creating a support ticket on these forums and letting us know.
