The IP address is required for IP bans, the username is not. Out system doesn't associate the banned IP with an account when it gets banned, so we need the actual IP that's blocked in order to know which one of the hundreds on the list we need to unblock.

 

For what it's worth, it was blocked for too many failed cPanel logins. For nearly all users, we recommend https://heliohost.org/login/ instead of the direct login since it won't get you blocked.
